Why the ISO file saved from disc is not the same as ISO made from disc?

When making ISO from disc, it performs as a 1:1 cloner.

But when saving ISO from an opened disc in editor, it performs as an ISO rebuilder. Files are the same, but file system is rebuilt which is different from the original disc's.
